CMS updated its Medicare Telehealth FAQ on October 8, 2026, adding billing guidance that applies as the current telehealth flexibilities wind down.
Two new claim modifiers are required beginning January 1, 2027: modifier BB for telehealth services furnished through a virtual platform owned by an entity that contracts with the physician or practitioner, and modifier BC for telehealth services furnished incident to a physician’s or practitioner’s professional service. CMS states that neither modifier carries payment implications. The FAQ also explains that rural health clinics and federally qualified health centers moved from HCPCS code G2025 to individual CPT or HCPCS codes, with the appropriate telehealth modifier, for non-behavioral telehealth beginning October 1, 2026.
The FAQ restates the January 1, 2028 change: except for behavioral health services, Medicare telehealth will again generally require the beneficiary to be in a medical facility located in a rural area, and physical therapists, occupational therapists, speech-language pathologists, and audiologists will no longer be able to furnish Medicare telehealth. Audio-only services in the beneficiary’s home remain available through December 31, 2027.
