CY 2027 Physician Fee Schedule Tightens Remote Monitoring Rules

CMS issued the calendar year (CY) 2027 Medicare Physician Fee Schedule final rule on November 4, 2026. Its remote monitoring section finalizes three of the four RPM and RTM policies the agency proposed in July.

Beginning January 1, 2027, remote therapeutic monitoring (RTM) services may be furnished only to established patients, and practitioners billing remote physiologic monitoring (RPM) or RTM must furnish a separately reportable initiating visit at the onset of those services. CMS also updated how RPM and RTM services are valued under the fee schedule. The fourth proposal, which would have limited payment to clinical staff employed by the practice rather than contractors, was delayed two years to January 1, 2029.

CMS also sought comment on bundling the RPM and RTM CPT codes and creating four new HCPCS G-codes for remote monitoring, and may consider those comments in future rulemaking. Separately, the rule implements Consolidated Appropriations Act, 2026 provisions that waive the in-person visit requirement for Medicare mental health telehealth through December 31, 2027 and keep RHC and FQHC payments for non-behavioral telehealth available through the same date.
